If you are a new patient to us, then book the medical marijuana New Patient (even if you already have the PA card from another doctor and this is a re-certification).
If you have been seen and certified by Dr. Gavin in the past, then book the medical marijuana F/U (follow up for your re-certification).

The Pennsylvania General Assembly passed the Medical Marijuana Act (Act 16) on April 17, 2016, legalizing the possession, sale, and consumption of medical cannabis. Only patients and caregivers who are Pennsylvania residents will be able to obtain medical cannabis from dispensaries in the state. Each patient must have his or her medical condition verified by a PDH-approved physician in order to acquire a written certification for medical cannabis. Steps to Get a Pennsylvania Medical Marijuana Card Through Azani Medical Spa
STEP 1: BOOK AN APPOINTMENT
COST FOR MEDICAL MARIJUANA CONSULT IS $200
Schedule an appointment to see Dr. Gavin at a time that is most convenient for you. There will be an evaluation of your ailments, and you make ask any questions you may have about medical marijuana treatment.

STEP 2: REGISTER WITH THE STATE OF PA
You will also need to register with the state using your drivers license ID number. The system is instantly verified and will give you a patient ID number. Dr. Gavin will upload this number and your records will be updated to reflect that you have been certified for medical marijuana treatment.
When registering with the PA DOH, you’ll use your driver’s license and upload information from it as the system is directly linked to the credentials on your ID. Make sure to follow the formatting instructions carefully, as the system demands that the information entered is exactly as it appears on your ID.
STEP 3: GET YOUR CARD
Lastly, to complete your application, register with the PA DOH and submit the $50 application fee. The state will mail you your card and you can begin purchasing marijuana legally from any Pennsylvania dispensary once you have your card in hand (typically 2-4 weeks).
Your certification is valid for the amount of time specified by Dr. Gavin, and you will need to renew it after its expiration by having another consultation with Dr. Gavin. Our staff will get in touch with you when your Pennsylvania medical marijuana card is approaching its expiration to help you setup a renewal consultation.

In addition to individuals seeking a medical marijuana card, Pennsylvania has set up a Patient and Caregivers’ Registry. Patients or caregivers must register for the MMP through the registry before seeking a physician’s certification, which confirms the patient has at least one of the required qualifying conditions to obtain a Pennsylvania medical marijuana card and legally use cannabis.

The Medical Marijuana Program of Pennsylvania is licensed under the Health Department of Pennsylvania. The Department of Health of Pennsylvania implemented and approved the Medical Marijuana Program of the State on April 17, 2016. Dispensaries are now available in PA and provide patients with a debilitating medical condition with access to medical marijuana.
Pennsylvania dispensaries offer cannabis in the form of oil, pills, tinctures, cream, ointments and liquids. Originally, PA dispensaries were prohibited to sell or purchase the dry form of the flower, but at certain places they are now providing it!
